Please note that this coverage excludes death by:

• Injuries intentionally inflicted on oneself or by another person
• Suicide
• Participating in an assault or crime
• Being intoxicated by alcohol, taking drugs or narcotics voluntarily unless prescribed by a qualified physician
• Committing any activity that is illegal or criminal in nature or associating with persons engaged in such activity
• The Certificate excludes and shall not provide coverage if the Insured dies from a Pre-existing Condition. Pre-existing Condition means an illness, disease or medical condition that has been diagnosed and treated by a physician:
  
  1. in the twelve (12) months before the Certificate’s effective date; and
     
  2. that caused the Insured’s death within twelve (12) months after the Certificate’s effective date.
